Make That School Physical Appointment Now
We would like to remind parents now is the time to get those school physicals out of the way. Avoid the last minute rush in August.
 
New Vaccination Requirements Grades 6-12
Students entering grades 6-12 must have appropriate documentation of the following vaccinations:
  • Tetanus
  • Diptheria
  • Acellular Pertussis (Tdap)
  • Meningococcal Conjugate (MCV4-Menactra)
  • 2 Chickenpox (varicella) appropriately spaced per CDC guidelines, or documentation history or laboratory evidence of immunity
