For Your Appointment
• Bring your current insurance card(s) to each appointment.
• Bring all current medication bottles with you to each appointment as well as a list of all over the counter medications or herbal supplements you are taking.
• For new patients, please arrive 5-10 minutes prior to your appointment time to complete new patient information sheets. The receptionist will make copies of your insurance card at that time.
• Kindly call 24 hours in advance if you are unable to keep an appointment. After three missed appointments in a year, without notification prior to appointment time, you may be dismissed from the practice.
• Our office does call the day prior to an appointment as a courtesy. However, on occasion these calls are not made so please do not depend on them to remember your appointment.
